Senior Oral Health Partnership

Senior Oral Health Partnership objectives (2024-2025): 
A. Create an asset map of San Diego County to help develop a comprehensive outreach and education/training plan. 
B. Plan, collect, analyze, and summarize SOHP related data 
C. Evaluate and improve seven of our partners' dental facilities for age-friendly access to dental care  
D. Increase number of senior patients (ages 60 years and above) that are medical patients of record that had at least one dental visit 
E. Develop standard senior oral health education material for healthcare providers and staff and conduct presentations/trainings 
F. Increase number of senior patients (ages 60 years and above) that had at least one dental visit, by developing standard oral health education material, conducting outreach, and providing oral health education and outreach in the community.

Required Activities (2024-2025):
A. Objective: Create an asset map of San Diego County to help develop a comprehensive outreach and education/training plan. 

1. Provide information on senior oral health stakeholders for the asset map. 

2. Provide aggregate senior data (i.e. utilization, insurance coverage, etc.) to help the partnership identify needs and trends. 

3. Collaborate with the partners to identify senior oral health linkages in the community. 

4. Assist in developing a plan to engage key stakeholders and partners in SOHP outreach and education. 

B. Objective: Plan, collect, analyze, and summarize SOHP related data. 

1. Assist HQP with developing the SOHP data collection, sharing and reporting processes. 

2. Submit required SOHP data monthly to HQP as noted in Exhibit C. Overview of Data Elements and Users. 

3. Participate in survey among the partnership members at  6 months and 12 months to determine satisfaction, areas for improvement, and Year 1 workplan ideas. 

C. Objective: Evaluate and improve seven of our partners' dental facilities  for age-friendly access to dental care.

1. Learn about the Institute for Healthcare Improvement (IHI) certification process and seek certification if feasible. 

2. Participate in age-friendly dental site assessments with the County’s Chief Dental Officer. 

3. Participate in monthly SOHP facilities work group meetings. 

4. Develop age-friendly modification plan for designated dental site(s).  

5. Make minor modifications to dental site(s) to improve accessibility. 

6. Develop plan for large modifications and to expand age-friendly modification plan to additional sites, if applicable. 

D. Objective: Increase number of medical senior patients (ages 60 years and above) that had at least one dental visit. 

1. Identify senior patients of record in need of oral healthcare, with focus on Medi/Medi patients. 

2. Establish and recruit Senior Oral Health Integration Coordinator (SOHIC) position.

3. Train new SOHIC in: motivational interviewing, unique oral health needs of seniors, and appointment scheduling. 

4. Conduct dental inreach to senior patients of record and schedule a comprehensive exam. 

5. Provide care coordination (appointment reminders, follow ups, referrals) to senior patients of record. 

6. Participate in bi-monthly SOHIC peer group meetings. 

E. Objective: Develop standard senior oral health education material for healthcare providers and staff and conduct presentations/trainings.  

1. Assist in the review of the educational material developed by the SOHP education and training work group. 

2. Coordinate senior oral health presentations/trainings with health center providers and staff. 

3. Conduct senior oral health presentations and trainings. 

4. Help develop education/training plans. 

F. Objective: Increase number of senior patients (ages 60 years and above) that had at least one dental visit by developing standard oral health education material, conducting outreach, and providing oral health education and outreach in the community. 

1. Assist in the review of the educational material developed by the SOHP education and training work group. 

2. Distribute SOHP educational material to senior population. 

3. Coordinate and conduct senior oral health education/outreach sessions.
