Case Management and Coordination of
Health Care
Public Health Nurse, Case Manager provides discharge planning and case management
for patients that are hospitalized. Services also include coordination of care for Elders
and/or patients with chronic illness. Yearly flu clinics in each reservation are facilitated by
the PHN as part of public health prevention. Our goal is to work with family and
communities to provide safe care at home and prevent future healthcare complications.
The Public Health Nurse works closely with the Diabetes Medical Team and the
Dementia/Cognitive Health Nurse to provide services in each tribal community, as well as
other service areas where IHC Native American’s reside. Medical care, prevention, and
case management are all services provided to maximize independence of individual care
needs, under the direction of our IHC Medical Providers.

Medi-Cal Peer Support Specialist Certification
IHC is providing training opportunities for community members, with lived experience in substance use recovery, to become a certified Medi-Cal Peer Support Specialist through the Tribal Opioid Response Program. 25 eligible participants will acquire the tools needed for an entry-level peer support specialist job opportunity over the span of 5 years (5 participants per year).

Registered Dietitian

Youth Change Makers Focus Group
We are an advisory group for Native youth ages 14-25 to collaborate on creating culturally relevant and age-appropriate mental health messaging. The messaging aims to normalize mental health conversations, foster awareness of available behavioral health services, and inspire help-seeking behavior amongst youth. The current campaign is called WE THRIVE.

Breast and Cervical Cancer Early Prevention
We provide classes on the signs and symptoms of breast and cervical cancers to the community. We also assist individuals with getting access to a free mammogram or pap smear within the county of San Diego. These services are free of charge and are to open to anyone that is unable to afford a copay, has no insurance/limited, and is migrant or undocumented.

Tuchily Healing Hearts - Mobile Crisis Response Team (MCRT)
Tuchily Healing Hearts- Mobile Crisis Response Team
Provides 24/7 crisis intervention and supportive services for individuals experiencing a mental health, substance use, or situational crisis, serving both Native and non-Native community members. Services include case management, transportation assistance, emergency shelter coordination, food support, and connection to essential resources.
Behavioral Health Bridge Housing (BHBH)
Behavioral Health Bridge Housing Program supports individuals experiencing or at risk of homelessness who have mental health or substance use concerns. The program provides emergency shelter, transportation assistance, food support, case management, and short- and long-term rental assistance with required participation in financial literacy, job readiness, and vocational services to promote long-term self-sufficiency.
State Opioid Response (SOR) – TAY Youth
The State Opioid Response program serves Transition Age Youth (TAY) experiencing or at risk of homelessness with substance use or mental health concerns. Services include rapid rehousing, treatment linkage, supportive services, and stabilization support.
Tribal Opioid Response (TOR)
The Tribal Opioid Response program provides community-based education, prevention, and outreach services focused on substance use awareness, harm reduction, and culturally responsive support.
